Summary
Horner syndrome arises from disruption of the oculosympathetic pathway and is classically identified by the triad of unilateral ptosis, miosis and facial anhidrosis. Lesions may occur at any point from central sympathetic neurons in the hypothalamus to post-ganglionic fibres in the carotid plexus. Early and accurate diagnosis hinges on a combination of clinical examination, pharmacological testing and imaging. Clinicians employ pharmacological agents such as cocaine and apraclonidine to confirm sympathetic denervation, while advances in digital pupillometry have enabled objective quantification of pupillary light reflex parameters and dilation lag. Imaging modalities ranging from magnetic resonance angiography to computed tomography are then tailored to the suspected lesion location, guiding targeted management. Therapeutic strategies centre on addressing the underlying cause—ranging from surgical resection of apical lung tumours to endovascular repair of carotid dissection—while symptomatic measures, including eyelid elevation and lubricants, improve patient comfort. A multidisciplinary approach, combining neurology, ophthalmology and radiology expertise, is essential to optimise outcomes and mitigate the risk of life-threatening aetiologies.
