Summary
Osteoarthritis, a leading cause of chronic joint pain and disability worldwide, is characterised by cartilage degradation, synovial inflammation and altered joint biomechanics. Hyaluronic acid (HA), a natural glycosaminoglycan present in synovial fluid and cartilage extracellular matrix, has emerged as a cornerstone of osteoarthritis therapy through viscosupplementation, chondroprotection and immunomodulation. Intra-articular delivery of HA aims to restore synovial fluid viscoelasticity, dampen inflammatory cytokine cascades and reduce oxidative stress within the joint. Beyond simple supplementation, chemical modification and formulation into injectable hydrogels enable sustained release of HA and co-delivered agents, promote endogenous cartilage matrix synthesis and recruit reparative cells. Recent advances encompass conjugates of HA with non-steroidal anti-inflammatory drugs, bioactive hydrogels that scavenge reactive oxygen species and mechanically active in vitro platforms to predict in vivo performance. These integrated strategies offer dual benefits of symptom relief and potential disease modification, highlighting HA’s versatility across global clinical settings.
