Immunological Factors in Reproductive Health
Summary
The interplay between the maternal immune system and reproductive processes is critical for successful conception, embryo implantation and maintenance of pregnancy. Innate immune cells, including uterine natural killer (NK) cells and macrophages, mediate trophoblast invasion and early placental development, while adaptive elements such as regulatory T cells (Tregs) ensure tolerance towards the semi-allogeneic foetus. A balanced profile of pro- and anti-inflammatory cytokines supports endometrial receptivity and vascular remodelling, whereas dysregulation of these mediators can contribute to implantation failure, recurrent pregnancy loss (RPL) and other reproductive disorders. Autoantibodies, complement activation and aberrant antigen presentation further shape pregnancy outcomes. Advances in diagnostic assays and immunomodulatory therapies are translating basic insights into predictive models and personalised interventions, with the potential to improve assisted reproduction success rates and reduce miscarriage risks worldwide.

Recent studies have applied machine learning to large cohorts of patients with RPL, integrating clinical features and routine laboratory data to stratify risk classes with high accuracy and objectivity. Such approaches facilitate early identification of individuals at greatest risk of recurrent loss and support data-driven clinical decision-making. In parallel, preclinical research in murine models has explored the therapeutic potential of tregitopes—specific self-derived IgG epitopes that expand Tregs and promote interleukin-10 production. Administration of these peptides in abortion-prone matings reduced foetal death rates and preserved a tolerogenic phenotype in antigen-presenting cells, pointing to a novel alternative to broad-spectrum immunoglobulin therapies.
