Summary

The COVID-19 pandemic has posed multifaceted challenges to the care of people with Parkinson’s disease. Infection with SARS-CoV-2 has been linked to transient or sustained worsening of motor and non-motor symptoms, particularly in those with advanced disease or delayed vaccination. Heightened systemic inflammation during acute illness has been associated with elevated proinflammatory markers and increased risk of severe outcomes, including prolonged hospital stays and higher mortality compared to non-Parkinson’s cohorts. Public health measures and resource reallocation disrupted routine consultations, rehabilitation and social support, giving rise to increased stress, anxiety and sleep disturbances. Rapid adoption of remote care models mitigated some access barriers but revealed stark socioeconomic disparities. Medication shortages and altered dosing regimens in resource-limited settings further compounded symptom management. These insights underscore the necessity of resilient, patient-centred healthcare frameworks that integrate preventive strategies, genetic risk assessment and equitable telemedicine solutions to safeguard long-term outcomes.

Recent studies have combined observational and genetic epidemiology to elucidate the interplay between Parkinson’s disease and COVID-19. A large cross-sectional survey reported that over 65% of people with Parkinson’s disease contracted COVID-19, with more than one third experiencing exacerbation of motor function. Key risk factors for infection included long disease duration (≥10 years) and prolonged interval since last vaccination (>12 months). Complementary Mendelian randomisation analyses demonstrated that underlying Parkinson’s disease predisposes to higher susceptibility to SARS-CoV-2 infection, while showing no direct causal effect on severity or hospitalisation. These findings highlight the importance of consistent immunisation, vigilant monitoring of high-risk patients and incorporation of genetic risk profiling into preventative care pathways.
