Summary
Impulse control disorders in Parkinson’s disease comprise a spectrum of compulsive behaviours—such as pathological gambling, hypersexuality, compulsive shopping and binge eating—that emerge disproportionately in patients receiving dopaminergic therapies. These disorders reflect a dysregulation of motivational circuits, particularly an overstimulation of the mesocorticolimbic pathway and alterations in frontostriatal connectivity. Epidemiological surveys estimate that up to 15 per cent of treated patients develop clinically significant symptoms, with risk heightened by factors such as younger age at onset, male gender, personal or family history of addictive behaviours and specific genetic variants affecting dopamine signalling. Clinically, impulse control disorders confer substantial social, financial and psychological burdens, often undermining quality of life. Management strategies centre on careful titration of dopaminergic medication, psychosocial interventions including cognitive–behavioural therapy and, in select cases, neuromodulation techniques. Advances in neuroimaging, genetics and behavioural profiling are guiding the emergence of stratified treatment paradigms tailored to individual neurobiological and clinical profiles.
