In vitro maturation (IVM) refers to the culture of immature human oocytes to the metaphase II stage outside the body, offering an alternative to conventional stimulated cycles. Advances in microfluidics and time-lapse imaging have enabled dynamic monitoring of meiotic progression, spindle assembly and cytoplasmic events, aiding the selection of developmentally competent oocytes. Although maturation and fertilisation rates remain lower than those achieved by in vivo-matured counterparts, IVM provides critical fertility preservation options for patients with hormone-sensitive cancers, polycystic ovary syndrome or poor ovarian reserve, and expands the donor oocyte pool. Ongoing work aims to optimise media composition, refine cryopreservation strategies post-IVM and understand the molecular drivers of meiotic competence.
