Summary

Inflammatory conditions of the female reproductive tract encompass a spectrum of disorders ranging from acute cervicitis and endometritis to salpingitis, pelvic inflammatory disease (PID) and tubo-ovarian abscess. These disorders are typically initiated by ascending infections of bacterial or, less commonly, viral and fungal origin. Common pathogens include Chlamydia trachomatis, Neisseria gonorrhoeae and a polymicrobial mix of anaerobic and facultative organisms. Inflammatory mediators and host immune responses play central roles in tissue damage and subsequent scarring, which may lead to chronic pelvic pain, tubal occlusion, ectopic pregnancy and infertility. Clinical diagnosis is often based on a combination of symptom assessment, physical examination and laboratory testing, yet reliable biomarkers remain under investigation. Standard management relies on broad-spectrum antibiotics, though rising antimicrobial resistance has prompted exploration of adjunct approaches such as local drug delivery systems and nutritional modulation. Globally, the burden of these diseases varies by region and socioeconomic factors, with significant implications for reproductive health, maternal morbidity and health-system costs. Ongoing research seeks to refine diagnostic criteria, optimise therapeutic regimens, explore preventive strategies and reduce long-term sequelae.

Pelvic inflammatory disease (PID): An inflammatory syndrome of the upper female genital tract involving the endometrium, fallopian tubes and adjacent pelvic structures following ascending infection.

Endometritis: Inflammation of the endometrial lining, often an early stage in the spectrum of PID.

Salpingitis: Inflammation of one or both fallopian tubes, frequently leading to tubal scarring and increased risk of ectopic pregnancy.

Tubo-ovarian abscess: A complex, pus-filled mass involving the fallopian tube and ovary, representing a severe complication of PID.

Hydrogels: Cross-linked polymer networks capable of absorbing large amounts of water, employed as drug-delivery matrices in the vaginal or urinary tract to prevent and treat infection.
