Integrative Approaches to Male Infertility Management

Male infertility arises from a complex interplay of genetic, environmental and lifestyle factors and affects an estimated 4–9 per cent of men worldwide. Integrative management seeks to combine conventional medical treatments, such as assisted reproductive techniques and surgery, with complementary strategies including herbal medicines, acupuncture, nutritional optimisation and psychosocial support. Central to this approach is the recognition that oxidative stress, hormonal imbalance and immune-mediated damage often co-exist in subfertile patients. Antioxidant supplementation aims to restore redox homeostasis and protect sperm DNA integrity, while tailored herbal formulations may modulate endocrine pathways and local testicular function. Acupuncture is employed to regulate neuroendocrine axes and improve pelvic blood flow. Recent advances in systems biology, network pharmacology and computational modelling are now being harnessed to deconvolute the active ingredients in traditional remedies and to predict molecular targets. By integrating omics technologies with rigorous clinical trials, this field aspires to offer personalised, evidence-based regimens that enhance semen parameters, hormonal profiles and ultimately pregnancy outcomes, while maintaining a favourable safety profile.

Recent studies have applied network pharmacology and molecular docking to elucidate how coupled herbal extracts exert beneficial effects on sperm quality. One investigation identified key bioactive compounds in a traditional medicinal pair that interact with androgen and oestrogen receptors, kinases and metabolic enzymes, implicating PI3K/Akt and AGE-RAGE pathways in the alleviation of oligoasthenozoospermia. Another foundational study demonstrated that echinacoside, a phenylethanoid glycoside, binds to hypothalamic androgen receptors to modulate luteinising hormone release and testosterone synthesis, leading to a marked increase in sperm count.
