Intestinal Protein Loss Mechanisms in Gastrointestinal Disorders
Summary
Intestinal protein loss encompasses a spectrum of mechanisms by which plasma proteins escape into the gastrointestinal lumen, leading to hypoalbuminaemia, oedema and immunodeficiency. Three principal pathways have been identified: disruption of the epithelial barrier in inflammatory conditions, dilation and rupture of lymphatic channels in lymphangiectasia, and elevated mucosal permeability secondary to portal hypertension or vascular injury. Inflammatory bowel diseases and infectious enteropathies drive exudative protein loss through cytokine-mediated damage to enterocytes and capillaries. Primary and secondary lymphangiectasia result from congenital or acquired obstruction of intestinal lymphatics, promoting leakage of chyle rich in albumin and immunoglobulins. In cirrhosis and congestive states, raised lymphatic pressure contributes to lymph leakage and worsened ascites. Clinically, protein loss manifests as peripheral oedema, diarrhoea and nutrient deficiencies. Diagnosis relies on a combination of endoscopic imaging, lymphatic studies, stool protein markers and histology. Management is tailored to the underlying mechanism and ranges from dietary interventions, notably low-fat regimens with medium-chain triglycerides, to medical therapies targeting lymphatic flow or inflammation, and surgical or interventional radiology procedures in refractory cases. Understanding these pathways underpins improved outcomes and informs development of targeted interventions worldwide.
