Summary
Osteoarthritis is a chronic joint disorder characterised by progressive cartilage degradation, synovial inflammation and subchondral bone remodelling. Conventional systemic treatments often fail to achieve sustained therapeutic concentrations within the joint and may cause systemic side effects. Intra-articular drug delivery systems offer a targeted approach by administering agents directly into the synovial cavity, maximising local bioavailability and reducing off-target exposure. Advances in formulation science have yielded a diverse toolkit of carriers—ranging from micrometre-scale depots to nanoscale vehicles—that can be engineered to penetrate dense cartilage matrices, adhere to specific extracellular components and release payloads in a controlled fashion. These systems include biodegradable polymeric microparticles, stimulus-responsive nanospheres, lipid-based vesicles and hybrid hydrogel constructs. By tuning parameters such as particle size, surface charge and polymer chemistry, researchers have achieved prolonged retention in synovial fluid, targeted chondrocyte uptake and on-demand release in response to pH, enzymatic activity or mechanical stress. The global significance of these technologies is underscored by their potential to deliver anti-inflammatory cytokine inhibitors, growth factors and disease-modifying osteoarthritis drugs (DMOADs) with only a single injection, thereby improving patient compliance and reducing healthcare burden. Practical applications range from viscosupplementation and analgesia to cartilage repair and immunomodulation, with preclinical models demonstrating cartilage protection, attenuated inflammatory gene expression and restoration of joint biomechanics. Integration of imaging agents and biomarker sensors further promises real-time monitoring of therapeutic distribution and response.
