Biologic preparations including platelet-rich plasma and mesenchymal cell derivatives are emerging as strategies to modulate tissue repair and inflammatory pathways. Advanced drug delivery systems, notably polymeric microsphere formulations, seek to extend therapeutic residence and reduce injection frequency. While intervention efficacy is well documented in the short term, long-term structural impact, optimal dosing intervals and safety considerations—such as infection risk and potential cartilage or meniscal alterations—continue to drive global research efforts towards more personalised, durable and disease-modifying regimens for diverse patient populations.

Recent studies have demonstrated that intra-articular corticosteroid injections in knee osteoarthritis result in transient alterations in joint structure. A nested case–control analysis using magnetic resonance imaging revealed a significant, though temporary, reduction in medial meniscal thickness and joint space width in the year of injection, while cartilage volume and patient-reported pain scores remained unchanged. These findings suggest that a single corticosteroid administration is structurally safe but prompts consideration of the clinical relevance of meniscal changes in long-term management.
