Summary

Intra-articular therapies for hip osteoarthritis aim to deliver pharmacological or biological agents directly into the joint space to alleviate pain, improve function and potentially modify disease processes. Established approaches include corticosteroid injections for rapid anti-inflammatory relief and viscosupplementation with hyaluronic acid to restore synovial fluid viscosity and joint lubrication. More recent innovations explore orthobiologic solutions such as platelet-rich plasma and mesenchymal stem cells, which seek to harness growth factors and regenerative potential. Experimental strategies have even incorporated metallic microparticles to modulate local inflammatory and proteomic profiles. Image guidance, most commonly ultrasound or fluoroscopy, is employed to ensure accurate needle placement and minimise complications. Collectively, these therapies offer non-surgical options for symptom management and may delay or reduce the need for arthroplasty in selected patients.

A pilot clinical study evaluated intra-articular injection of metallic gold microparticles suspended in hyaluronic acid in patients with moderate to severe hip osteoarthritis. Over a two-year follow-up, participants reported marked reductions in pain and improvements in stiffness and physical function as measured by the Western Ontario and McMaster Universities Arthritis Index. Early analysis of synovial fluid and serum revealed significant shifts in proteomic biomarkers eight weeks post-injection, suggesting a sustained biological effect. Safety was acceptable, with no serious device-related adverse events. These findings establish proof of concept and support a future placebo-controlled randomised trial of gold microparticles combined with hyaluronic acid in hip osteoarthritis.
