Intra-Articular Treatment Strategies for Joint Inflammation

Summary

Intra-articular interventions aim to deliver therapeutic agents directly into the joint space to mitigate inflammation and promote tissue repair in conditions such as osteoarthritis and inflammatory arthritis. Strategies include pharmacological injections—such as glucocorticoids, local anaesthetics, hyaluronic acid and novel drug-carrying hydrogels—and biological therapies including mesenchymal stem cells or their secretome. The rationale for local delivery is to achieve high intra-articular drug concentrations with minimal systemic exposure, reducing adverse effects while targeting key inflammatory mediators, chondrocytes and synovial macrophages. Recent advances have focused on optimising drug combinations to enhance anti-inflammatory and chondroprotective actions, refining pharmacokinetic and pharmacodynamic profiles to prolong residence time, and exploring regenerative approaches that modulate the joint microenvironment through paracrine signalling. Preclinical and clinical studies in animal models have elucidated mechanisms by which intra-articular treatments regulate cytokine networks, extracellular matrix turnover and haemostatic pathways within synovial fluid. The global burden of joint disease drives ongoing research into off-the-shelf therapeutics, sustained-release formulations and precision delivery systems to alleviate pain, preserve function and defer surgical interventions.
