Summary
Intraindividual sleep variability refers to the night-to-night fluctuations in sleep timing, duration and continuity within the same person. Unlike average sleep duration, variability captures irregular patterns such as shifting bedtimes, inconsistent wake-up times and uneven total sleep time. These irregularities can disrupt circadian rhythms, alter hormonal secretions and impair metabolic processes. Emerging evidence links greater variability with adverse cardiometabolic outcomes, heightened mortality risk, poorer mental health and diminished cognitive performance. Mechanistic pathways include circadian misalignment, altered autonomic balance and inflammatory activation. At a global level, populations exposed to shift work, social jetlag or lifestyle stressors exhibit elevated sleep variability and suffer disproportionate health burdens. Practical applications involve wearable monitors to quantify variability, behavioural interventions to promote consistent schedules and light-based therapies to restore circadian alignment. One foundational investigation introduced the Sleep Regularity Index (SRI) as a robust measure of day-to-day consistency. It demonstrated that higher irregularity in sleep–wake timing among healthy young adults was associated with delayed circadian phase and lower academic performance, highlighting a direct link between sleep regularity and cognitive outcomes. Another study validated the SRI in older adults, revealing that greater sleep irregularity correlated with elevated cardiometabolic risk factors, including hypertension, fasting glucose and obesity, independently of sleep duration. A further analysis of large-scale wearable data collected during pandemic lockdowns showed that reduced variability in midsleep timing and total sleep time was accompanied by a decline in resting heart rate, indicating improved autonomic stability when social schedules were relaxed.
