Intratumoral Immunotherapy Strategies for Cancer Treatment
Summary
Intratumoral immunotherapy harnesses the patient’s own tumour as an in situ source of antigen by directly administering immune-modulating agents into lesions. This approach aims to reprogramme an immunosuppressive tumour microenvironment into one that supports antigen presentation, effector cell recruitment and systemic antitumour immunity. Strategies include the delivery of cytokines, toll-like receptor agonists, engineered nanoparticles, dendritic cell formulations and viral vectors directly into tumours. By concentrating immunostimulatory payloads at the disease site and minimising systemic exposure, intratumoral administration can enhance local retention, promote immunogenic cell death and trigger broader T-cell responses against distal metastases. Contemporary research seeks to optimise pharmacokinetics within the tumour matrix, identify combinations that synergise with checkpoint blockade or radiation, and develop materials that co-localise hyperthermia or adjuvancy to amplify therapeutic efficacy.

Recent studies have established a quantitative framework for the pharmacokinetics of locally administered cytokines. By modulating molecular size and affinity for extracellular matrix components, investigators demonstrated that extended tumour retention of interleukin-2 markedly improves disease control in preclinical models, informing rational design of protein-based immunotherapies. In parallel, a bacterial biomineralization-derived nanomaterial combining a gold core with surface-bound microbial components was shown to engage multiple pattern recognition receptors and, upon laser-induced hyperthermia, to enhance vascular perfusion, effector lymphocyte infiltration and cytokine release. This multifunctional construct selectively amplifies local immunity and potentiates the effects of established chemo- and immunotherapeutic regimens in murine tumour models.
