Summary

Irritability in young people manifests as a heightened proneness to anger, frustration and temper outbursts, often accompanied by persistent negative mood. Historically viewed as a symptom dimension across mood and behavioural disorders, irritability has gained prominence as both a transdiagnostic construct and a potential marker of emerging psychopathology. In childhood and adolescence, chronic irritability may herald later depressive and anxiety disorders, while episodic irritability can signal bipolar or mood‐dysregulation phenotypes. Disruptive Mood Dysregulation Disorder (DMDD) was introduced to capture severe, non-episodic irritability characterised by frequent temper outbursts and a persistently irritable or angry mood between episodes. Across clinical and community samples, irritability correlates with functional impairment in home, school and peer contexts, underscoring its public health importance.

Neurobiological investigations point to altered limbic and prefrontal circuit dynamics in irritable youth, with aberrant amygdala reactivity to emotional faces and diminished regulatory control in anterior cingulate and orbitofrontal regions. Genetic studies further suggest shared heritability with attention-deficit/hyperactivity disorder (ADHD) and mood disorders, with copy number variants contributing to individual differences in irritability presentations. Assessment approaches have evolved to include brief parent- and self-report measures, yet informant discrepancies remain a challenge, reflecting divergent interpretations of observable behaviours versus internal states. Contemporary frameworks emphasise irritability as a dimensional trait with both state-like fluctuations and trait-like stability, calling for early identification and targeted interventions that span pharmacological, behavioural and family-based strategies.
