Summary

Joint arthroplasty has transformed the management of end-stage osteoarthritis and other degenerative joint conditions by reliably reducing pain, restoring mobility and improving quality of life. Advances in implant design and surgical technique have extended implant longevity, with modern prosthetic materials achieving survival rates exceeding 90% at ten years. Patient outcomes are now assessed not only by implant survivorship but increasingly by patient-reported outcome measures (PROMs) that capture pain relief, functional capacity and health-related quality of life. Rehabilitation has evolved from standardised, time-based protocols to individualised programmes that combine early mobilisation, strength training and neuromuscular re-education. Preoperative optimisation—or prehabilitation—addresses modifiable risk factors such as muscle weakness, obesity and smoking, and is associated with shorter length of stay and faster functional recovery. Postoperative rehabilitation integrates physiotherapy with digital monitoring tools and wearable sensors to provide objective gait and activity metrics, enabling tailored progression and remote supervision. Emerging approaches include enhanced recovery pathways, tele-rehabilitation and immersive technologies that aim to maximise patient engagement and optimise long-term outcomes. Despite these advances, disparities in access and outcome persist across socioeconomic groups, emphasising the need for equity in service delivery and ongoing innovation in rehabilitation strategies.
