Summary
Juvenile idiopathic arthritis (JIA) represents a spectrum of chronic paediatric rheumatic diseases marked by persistent joint inflammation of unknown aetiology. Classification is based on the number of joints affected, systemic features and serological markers, yielding oligoarticular, polyarticular, systemic, psoriatic and enthesitis-related subtypes. Assessment combines clinical evaluation with standardised composite indices, most notably the Juvenile Disease Activity Score (JADAS), alongside imaging techniques such as ultrasound and magnetic resonance to detect subclinical synovitis. Patient-reported outcome measures have gained prominence to capture pain, function and quality of life. Management has evolved towards a treat-to-target paradigm in which tight control of inflammation and regular reassessment guide therapeutic escalation. Conventional synthetic DMARDs, chiefly methotrexate, remain first line for peripheral disease, often supplemented by intra-articular corticosteroids. The advent of biologic agents targeting tumour necrosis factor, interleukin-1 and interleukin-6 has transformed outcomes, particularly in refractory or systemic cases. A multidisciplinary team including paediatric rheumatologists, ophthalmologists, physiotherapists and psychologists is essential to address musculoskeletal health, prevent complications such as uveitis, support growth and optimise psychosocial development. Emerging biomarker research and digital health tools aim to refine individual risk stratification, predict response to therapy and enhance self-management.
