Kienböck's Disease Diagnosis and Treatment Strategies

Kienböck’s disease is characterised by avascular necrosis of the lunate bone, leading to progressive wrist pain, reduced range of motion and eventual carpal collapse. Diagnosis relies on clinical assessment of dorsal wrist pain aggravated by loading, combined with imaging studies. Plain radiographs are used to determine staging according to the Lichtman classification, while MRI and CT scans allow early detection of vascular compromise and bone architecture changes. Ulnar variance is assessed to guide joint-level load redistribution strategies. Treatment strategies span from non-operative unloading in early stages to surgical interventions aimed at revascularisation, decompression and mechanical realignment. Joint-preserving procedures include radial shortening osteotomy for negative ulnar variance, capitate shortening or distal radius osteotomy in neutral or positive variance, and vascularised bone grafting to restore lunate perfusion. In more advanced stages, limited intercarpal arthrodesis or proximal row carpectomy may be performed to stabilise carpal kinematics and relieve pain. For end-stage disease, total wrist fusion or arthroplasty options offer durable pain relief at the expense of wrist motion. Emerging technologies such as customised prosthetic implants and biomechanically optimised external fixators aim to bridge gaps between reconstructive and salvage procedures.

A novel biomechanical study introduced a capitate-based Ilizarov external fixator designed to unload the necrotic lunate in late-stage osteonecrosis. Anatomical measurements from cadaveric specimens and patient radiographs informed two fixator designs tailored to capitate morphology. Dynamic fatigue and pull-out resistance testing demonstrated superior stability and lower displacement compared with a standard external device. By aligning fixation forces along the capitate’s longitudinal axis, the approach promises a minimally invasive method to off-load the lunate and promote revascularisation. Further clinical evaluation is under way to confirm functional outcomes and bone healing rates.
