Summary
Kinesiophobia, defined as an excessive, irrational and debilitating fear of physical movement and activity arising from a feeling of vulnerability to painful injury or re-injury, is increasingly recognised as a critical barrier to effective management of chronic musculoskeletal disorders. In conditions such as osteoarthritis, chronic low back pain and persistent neck pain, this fear leads to avoidance behaviours, muscular deconditioning and altered movement patterns, further perpetuating pain and disability. Contemporary pain management approaches adopt a biopsychosocial framework in which kinesiophobia is addressed alongside nociceptive and inflammatory processes. Strategies include graded exposure to feared movements, cognitive-behavioural therapy to restructure maladaptive beliefs, and tailored exercise prescriptions to rebuild confidence and physical capacity. Interdisciplinary rehabilitation teams, encompassing physiotherapists, psychologists and pain specialists, work collaboratively to recalibrate the individual’s pain expectations and restore functional activity. Globally, implementation of these evidence-based interventions has demonstrated improvements in physical performance, quality of life and long-term adherence to therapeutic exercise, underscoring the urgent need for early identification and targeted treatment of kinesiophobia within chronic musculoskeletal care pathways.
