Knee Osteoarthritis Assessment and Management Techniques

Knee osteoarthritis is a global health concern characterised by progressive degeneration of articular cartilage, subchondral bone remodelling and synovial inflammation, leading to pain, stiffness and functional impairment. Assessment strategies combine clinical examination with patient-reported outcome measures, performance-based tests and imaging modalities. Standardised questionnaires such as the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) and the Knee injury and Osteoarthritis Outcome Score (KOOS) quantify pain, stiffness and functional limitation, while biomechanical assessments include gait analysis and functional performance tests to detect early mechanical alterations. Imaging ranges from plain radiography with Kellgren–Lawrence grading to advanced magnetic resonance techniques that visualise cartilage, bone marrow lesions and synovitis. Biomarker research seeks biochemical indices of cartilage degradation and inflammation to predict progression. Management is multi-modal and tailored to disease severity: first-line interventions comprise patient education, weight management and structured exercise programmes that target muscle strength and joint flexibility. Pharmacological therapies include analgesics, non-steroidal anti-inflammatory drugs and intra-articular injections. In advanced cases, surgical options such as osteotomy or total knee replacement restore function and quality of life. Emerging approaches encompass regenerative injections, digital health tools for remote monitoring and predictive algorithms to stratify progression risk, all aiming to personalise care and mitigate the societal burden of knee osteoarthritis.
