Light Therapy Interventions in Cognitive Impairment

Summary

Light therapy has emerged as a non-pharmacological strategy to address cognitive decline and neuropsychiatric symptoms in conditions such as mild cognitive impairment, Alzheimer’s disease and other dementias. By exposing individuals to tailored intensities and spectra of light, it is possible to reinforce circadian entrainment, augment sleep quality and modulate mood and cognitive performance. Interventions range from brief daily sessions with specialised light boxes to ambient dynamic lighting systems integrated into care environments. Optimisation of timing, wavelength and duration of exposure is informed by an understanding of non-visual photoreception through intrinsically photosensitive retinal ganglion cells, which project to the suprachiasmatic nucleus, the master clock governing physiological rhythms. Clinically, light therapy has been applied to alleviate depressive symptoms, reduce agitation and improve memory and attention. Technological advances now enable blue-enriched white light schedules that align with individual melatonin onset times, while lightweight wearable sensors and actigraphy facilitate objective monitoring of sleep and rest–activity patterns. Globally, this approach offers a low-risk adjunct to existing therapies, with particular relevance to care home settings and community-dwelling older adults. Continued work seeks to determine optimal dosing parameters and to integrate personalised light interventions into routine care.

Recent studies have shown that morning exposure to blue-enriched white light timed to each individual’s dim light melatonin onset can yield measurable gains in both sleep quality and cognitive function in patients with mild to moderate Alzheimer’s disease. Participants who received daily one-hour sessions of controlled blue-enriched illumination exhibited improvements in global sleep scores at follow-up and demonstrated modest but significant increases in standard cognitive screening measures. These findings support the notion that precisely timed short-wavelength light can strengthen circadian alignment and may slow aspects of cognitive decline.
