Osteoarthritis is increasingly recognised not merely as a mechanical wear-and-tear disorder but as a metabolic disease in which lipid homeostasis plays a central role. Lipids such as cholesterol, fatty acids and lipoproteins influence cartilage health, synovial membrane function and subchondral bone remodelling. Aberrant lipid accumulation in chondrocytes and synovial tissues can trigger inflammatory mediators, oxidative stress and matrix-degrading enzymes, accelerating cartilage erosion and osteophyte formation. Conversely, protective lipid fractions, notably high-density lipoproteins, may counteract inflammatory cascades and preserve joint integrity. Advances in molecular biology have uncovered pathways by which lipid species modulate gene expression, influence cellular senescence and alter extracellular matrix composition. Therapeutic strategies targeting lipid metabolism—ranging from lifestyle interventions and statins to RNA-based and small-molecule modulators—offer prospects for disease modification. A deeper understanding of lipid networks in joint tissues holds promise for stratified treatments that address metabolic drivers of osteoarthritis alongside mechanical factors, with global health implications for ageing populations and the rising prevalence of obesity and metabolic syndrome.

A long-term population study has demonstrated an inverse relationship between high-density lipoprotein cholesterol levels and the incidence of radiographic hand osteoarthritis over an 11-year period, suggesting a protective role of this lipid fraction in joint preservation. Analyses of serum lipids revealed that elevated triglyceride concentrations were linked to increased osteoarthritis risk, whereas total and low-density lipoprotein cholesterol showed no consistent association, highlighting the importance of lipid subtypes in disease susceptibility.
