Locomotive Syndrome Assessment and Management
Summary
Locomotive syndrome encompasses the decline in mobility and musculoskeletal function that presages loss of independence in daily activities. It is defined by impaired walking, standing and balance due to disorders of the bones, joints, intervertebral discs and muscles. Early identification relies on a combination of self-reported questionnaires and simple performance tests, notably the Geriatric Locomotive Function Scale-25 (GLFS-25), the two-step test and the stand-up test. Management strategies integrate tailored exercise regimens—including strength, balance and dynamic training—nutritional guidance to optimise body composition, and risk-factor modification such as weight control and smoking cessation. Recent advances have introduced digital tools for automated gait assessment and statistical modelling to stratify trajectories of functional decline. A multidisciplinary approach engaging orthopaedic clinicians, physiotherapists and public health professionals is essential to arrest progression, support rehabilitation and sustain mobility across the lifespan.

Recent studies have clarified the relationship between intra-abdominal adiposity and early locomotive impairment. A large cross-sectional analysis covering adults aged 20 to 85 years demonstrated that higher visceral fat area was consistently associated with the initial stage of locomotive syndrome. This association was independent of body mass index and muscle mass indices, and was evident across both younger and older cohorts. The findings underscore the importance of diet and exercise interventions aimed at reducing visceral adiposity to prevent or ameliorate early functional decline in diverse age groups.
