Loudness Dependence of Auditory Evoked Potentials in Affective Disorders
Summary
The loudness dependence of auditory evoked potentials (LDAEP) describes how the amplitude of cortical responses to sound varies with stimulus intensity. Recorded via electroencephalography, it serves as a non-invasive proxy for central serotonergic neurotransmission: a steeper amplitude–intensity slope reflects lower serotonergic activity, whereas a flatter slope indicates higher activity. In affective disorders—including major depressive disorder, bipolar disorder and anxiety disorders—serotonergic dysfunction is implicated in mood instability, emotional reactivity and impulsivity. LDAEP has therefore been investigated both as a diagnostic biomarker and as a predictor of response to serotonergic antidepressants. Research has linked altered LDAEP to symptom severity, impulsive aggression and suicide risk, and has explored its integration with advanced source-imaging and machine-learning methods. Collectively, these findings underscore LDAEP’s potential to inform personalised treatment strategies and to elucidate fundamental neurophysiological mechanisms underlying affective dysregulation across diverse clinical populations.

Recent studies have demonstrated that individuals with heightened LDAEP exhibit greater emotional sensitivity and impulsivity. In healthy volunteers, high LDAEP was associated with increased scores on measures of impulsivity and affective lability, as well as augmented cortical responses in prefrontal and occipital regions during inhibitory control tasks. These findings suggest that LDAEP may index variability in emotion regulation circuits beyond simple sensory processing. In patients with major depressive disorder, comparative analyses of those with suicide attempts, suicidal ideation and healthy controls revealed no consistent difference in LDAEP once psychological factors were controlled. This work indicates that although serotonergic dysfunction contributes to suicidal behaviour, LDAEP alone may not distinguish high-risk individuals without accounting for broader psychological profiles.
