Malakoplakia Pathogenesis and Clinical Management
Summary
Malakoplakia is an uncommon chronic granulomatous disorder arising from defective macrophage bactericidal activity. A primary defect in phagolysosomal processing leads to accumulation of partially digested bacteria and characteristic intracytoplasmic inclusions. These Michaelis–Gutmann bodies consist of calcium and iron deposits within enlarged histiocytes and signify impaired lysosomal killing. The urinary tract is most frequently affected, but gastrointestinal, cutaneous, soft tissue and other systemic sites may be involved, especially in immunocompromised hosts. Clinical presentation ranges from mass lesions mimicking neoplasia to nonspecific symptoms such as pain, diarrhoea or urinary obstruction. Diagnosis requires histopathological confirmation of Michaelis–Gutmann bodies within sheets of CD68-positive histiocytes. Management is guided by lesion location, severity and host immune status. Prolonged antibiotic regimens with agents that achieve high intracellular concentrations—particularly fluoroquinolones—form the backbone of therapy. Adjunctive measures, including cholinergic agonists and vitamin C to enhance phagolysosomal function, have shown benefit in some reports. Surgical resection remains necessary for extensive or obstructive lesions. Early recognition and targeted therapy can prevent irreversible organ damage and reduce the need for radical interventions.
