Mammalian Bite Injury Epidemiology and Infection Management
Summary
Mammalian bite injuries constitute a widespread public health challenge, with dog and cat bites accounting for the majority of cases worldwide. Incidence varies by region, season and demographic factors, but many studies report rising trends in adult bite admissions and substantial direct healthcare costs. Common bacterial pathogens include Pasteurella multocida, Capnocytophaga spp. and Staphylococcus aureus, which can lead to local wound infections, cellulitis or, rarely, systemic sepsis. Management hinges on prompt wound assessment, adequate debridement and irrigation, and judicious use of prophylactic antimicrobials guided by anatomical site and patient risk factors. Tetanus status must be reviewed and updated as required, especially in deep or contaminated wounds, and rabies prophylaxis remains critical in endemic areas or following bites from wildlife reservoirs. Advances in molecular diagnostics have improved identification of zoonotic organisms, enabling targeted therapy and epidemiological surveillance. Preventive strategies encompass community education on animal handling, environmental and behavioural risk mitigation, and integration of One Health approaches to control stray animal populations, vaccination campaigns and intersectoral coordination.

Recent analysis of over sixty‐nine thousand dog bite reports has revealed a strong association between higher ambient temperatures, increased ozone levels and ultraviolet irradiation with elevated bite rates. This work highlights the need to consider environmental stressors on animal behaviour and human–animal interactions when planning public health interventions. A longitudinal study of hospital admissions over two decades in England demonstrated that rising bite admission rates are driven entirely by adult cases, resulting in escalating direct healthcare expenditure. These findings underscore the importance of demographic‐specific prevention programmes and the allocation of resources to emergency and surgical services.

Capnocytophaga: A genus of gram-negative bacteria commonly found in canine and feline oral flora that can cause severe human infections following mammalian bites.
Debridement: The surgical or mechanical removal of devitalised or contaminated tissue to reduce infection risk and promote wound healing.
PCR-RFLP: Polymerase chain reaction-restriction fragment length polymorphism, a molecular technique that amplifies DNA and distinguishes species by unique patterns generated by enzyme digestion.
Prophylactic antibiotic: Antimicrobial therapy administered to prevent infection in a clean or minimally contaminated wound rather than to treat an established infection.
