Management and Outcomes of Tetanus Cases

Tetanus is a life-threatening neuromuscular disorder caused by Clostridium tetani that is largely preventable through vaccination yet continues to pose a risk in settings with suboptimal immunisation and limited critical care. Management comprises prevention by active immunisation with primary and booster doses, post-exposure prophylaxis with antitoxin, meticulous wound debridement and antibiotic therapy to eradicate local bacterial proliferation. Once clinical tetanus develops, treatment focuses on neutralising unbound toxin, controlling muscle rigidity and spasms using sedatives and neuromuscular blockers, managing autonomic instability with cardiovascular support, and providing respiratory assistance via mechanical ventilation as necessary. The introduction of standardised intensive care protocols and access to ventilatory support has substantially reduced mortality in high-income settings, whereas in many low- and middle-income regions the case-fatality rate remains elevated owing to delayed presentation, limited ventilator availability and under-recognised disease. Prognostic factors include patient age, shorter interval from wound to symptom onset, and occurrence of complications such as infections or severe autonomic dysfunction. Broad implementation of holistic management strategies, together with sustained immunisation campaigns, is critical to improving global outcomes and reducing preventable deaths.
