Summary

Congenital branchial anomalies encompass a spectrum of embryonic defects arising from the second, third or fourth branchial apparatus, presenting as cysts, sinuses or fistulae in the neck. These anomalies often manifest with recurrent cervical infections, abscess formation and, in many cases, involvement of the thyroid gland. Suppurative thyroiditis may complicate branchial cleft sinuses, leading to acute abscesses that demand prompt medical and surgical intervention. Optimal management requires a combination of accurate imaging, targeted antibiotic therapy, drainage where appropriate and definitive excision of anomalous tracts. Advances in endoscopic techniques and refined surgical algorithms have reduced morbidity and recurrence, while neonatal and paediatric imaging protocols have improved early recognition. Multidisciplinary collaboration between otolaryngologists, radiologists and endocrinologists ensures timely diagnosis and tailored treatment, minimising the risk of complications and preserving thyroid function.

Branchial cleft anomalies: Congenital defects of the embryonic branchial apparatus resulting in cysts, sinuses or fistulae.
