Management of Small Bowel Obstruction

Small bowel obstruction (SBO) represents a critical surgical emergency defined by interruption of intestinal flow. It arises most commonly from postoperative adhesions, but may also be caused by hernias, tumours, volvulus or inflammatory strictures. Prompt recognition through clinical assessment, biochemical analysis and imaging is essential to distinguish uncomplicated cases from those with strangulation or ischaemia. Initial management centres on resuscitation with intravenous fluids, electrolyte correction and gastrointestinal decompression via nasogastric or long-tube drainage. Water-soluble contrast agents serve both diagnostic and therapeutic roles by predicting resolution and reducing the need for operative intervention. Non-operative management may be pursued safely in the absence of peritonitis, systemic signs of sepsis or radiological indicators of vascular compromise. When conservative measures fail or complications arise, surgical intervention is indicated. Laparoscopic adhesiolysis has gained traction in selected patients with a single obstructing band and minimal prior surgery, offering reduced morbidity, shorter hospital stay and faster return of bowel function compared with open laparotomy. Elective adhesiolysis in patients with chronic adhesion-related pain can lower the long-term risk of adhesive SBO. Perioperative innovations include dual-energy computed tomography to stratify ischaemia risk and predictive nomograms that integrate body composition and inflammatory markers to forecast postoperative morbidity. Machine-learning models further aim to identify individuals at heightened risk of complications following SBO surgery, facilitating early personalised interventions. Preventive strategies such as minimally invasive techniques and adhesion barriers are integral to reducing SBO incidence and recurrence. Collectively, these advances underscore a shift towards precision management, balancing conservative care with tailored surgical approaches to optimise outcomes globally.
