Management of Sudden Sensorineural Hearing Loss

Summary

Sudden sensorineural hearing loss (SSNHL) is typically defined as a rapid decline in auditory function of at least 30 dB across three contiguous frequencies occurring within 72 hours. Prompt recognition and intervention are pivotal to optimise recovery. Empirical systemic corticosteroids remain the cornerstone of initial treatment, aiming to reduce cochlear inflammation and microvascular compromise. In cases where systemic therapy is contraindicated or yields suboptimal response, intratympanic steroid injection offers targeted delivery to the inner ear. Adjunctive measures include hyperbaric oxygen therapy to enhance cochlear oxygenation and address ischaemic components. Supportive care often encompasses antiviral agents, vasodilators or anticoagulants when vascular or viral aetiologies are suspected, though their roles remain debated. Prognostic factors such as degree of hearing loss at presentation, presence of vertigo, comorbid cardiovascular or metabolic disease and timing of treatment strongly influence outcome. Rehabilitation strategies, including hearing aids or cochlear implantation, are considered for patients with persistent severe loss. A multidisciplinary approach, integrating otolaryngology, audiology and rehabilitation services, underpins contemporary management and seeks to personalise therapy based on risk factors and early response.

Analysis of a large cohort of patients in China has delineated the interplay between cardiovascular and metabolic comorbidities and the severity of SSNHL. Patients presenting with vertigo, hypertension, diabetes mellitus and elevated triglycerides exhibited more profound initial hearing loss. The study highlighted that metabolic derangements and hyperlipidaemia correlate with poorer auditory outcomes, underscoring the need to assess and manage systemic risk factors alongside standard corticosteroid regimens. These findings have encouraged investigators to consider stratified treatment protocols that incorporate metabolic control and close monitoring of high-risk patients to improve overall prognosis.
