Summary
Uterine fibroids and leiomyomas are benign smooth muscle tumours affecting a substantial proportion of women of reproductive age worldwide. Clinical manifestations vary from asymptomatic cases to heavy menstrual bleeding, pelvic pain and pressure symptoms that impair quality of life. Management strategies span conservative monitoring, pharmacological interventions, image‐guided procedures and surgery. Pharmacological options include gonadotrophin‐releasing hormone analogues, selective progesterone receptor modulators and aromatase inhibitors, which aim to reduce bleeding and tumour volume. Minimally invasive techniques such as uterine artery embolisation and high‐intensity focused ultrasound offer uterine‐preserving alternatives with shorter recovery times. Myomectomy remains the standard for women desiring future fertility, whereas hysterectomy provides definitive treatment. A growing emphasis on personalised care integrates symptom severity, fertility desires and molecular characteristics of individual tumours to guide intervention choice and optimise long‐term outcomes.

Recent studies have engineered precise MED12 hotspot mutations in smooth muscle cells to establish a three‐dimensional cellular model that faithfully reproduces fibroid characteristics, including altered metabolic pathways, enhanced proliferation and extensive extracellular matrix deposition. This platform enables functional dissection of mutation‐driven gene networks and preclinical screening of targeted therapies. Complementary genetic research has applied large‐scale genome‐wide association and epidemiological analyses to identify novel susceptibility loci for uterine leiomyomata and overlapping risk variants with endometriosis. These findings elucidate shared pathogenic mechanisms, suggest biomarkers for risk stratification and open avenues for genotype‐guided management strategies.
