Management of Vestibular Schwannomas and Related Outcomes
Summary
Vestibular schwannomas are benign, slow-growing neoplasms of Schwann cell origin arising at the cerebellopontine angle. Management strategies range from active surveillance with serial imaging for small, asymptomatic lesions to surgical resection and radiotherapy for tumours demonstrating growth or causing significant symptoms. Microsurgical approaches—including translabyrinthine, retrosigmoid and middle fossa techniques—aim to achieve complete tumour removal while preserving facial nerve function and, where possible, hearing. Stereotactic radiosurgery and fractionated stereotactic radiotherapy offer non-invasive tumour control, with progression-free survival rates typically above 90 per cent in properly selected patients. Hearing preservation, facial nerve integrity and quality of life are primary outcome measures and depend on tumour size, location, growth rate and patient factors such as age and baseline function. Emerging research has illuminated the tumour microenvironment and molecular pathways driving growth, opening avenues for targeted medical therapies. Technological advances in intraoperative nerve mapping, high-resolution imaging biomarkers and molecular profiling are reshaping decision-making, enabling individualised treatment and optimising functional outcomes on a global scale.

Recent studies employing single-cell multi-omics have uncovered a nerve injury–like state within the vestibular schwannoma microenvironment. Diverse populations of Schwann cells, stromal elements and immune cells were profiled, revealing that repair-like Schwann cells recruit myeloid populations via CSF1 signalling. This process correlates with larger tumour size and suggests new molecular targets to limit growth. Another foundational investigation demonstrated that factors secreted by vestibular schwannomas can directly damage cochlear explants. Tumour-derived TNFα was identified as ototoxic, whereas FGF2 exerted a protective effect. Neutralising TNFα in conditioned media partially prevented hair cell loss, pointing to intervention strategies aimed at mitigating sensorineural hearing loss in patients.
