Management of White Spot Lesions in Orthodontics

Summary

White spot lesions represent the earliest visible sign of enamel demineralisation during fixed orthodontic treatment. These subsurface enamel defects arise from prolonged plaque accumulation around brackets and bands, leading to acid production by cariogenic bacteria. Management requires a multipronged approach encompassing rigorous oral hygiene, regular professional monitoring and the application of preventive or reparative agents. Prophylactic strategies include patient education on effective brushing techniques, adjunctive use of fluoride toothpastes, mouthwashes and varnishes, as well as the application of sealants or protective coatings to bracket interfaces. When demineralisation is identified, minimally invasive treatments such as casein phosphopeptide-amorphous calcium phosphate complexes, self-assembling peptides and fluoride-containing dental mousse can arrest lesion progression and promote remineralisation. Emerging materials are designed to deliver sustained release of antibacterial or remineralising agents directly in situ. A coordinated effort between orthodontists, hygienists and restorative specialists is essential to tailor interventions, optimise aesthetic outcomes and preserve long-term dental health.

Recent studies have demonstrated that combining self-assembling peptide P11-4 with a single application of fluoride varnish significantly enhances remineralisation of early lesions adjacent to brackets, outperforming fluoride alone as measured by quantitative light-induced fluorescence over 30 days. Another body of work has evaluated sealants and bonding materials in clinical trials, revealing that protective coatings around brackets reduce the initiation of white spot lesions. Interestingly, materials releasing fluoride did not show superior efficacy compared to non-fluoride variants, suggesting that the physical barrier afforded by sealants may be the key factor in prevention. Both investigations underscore the value of targeted enamel coatings to intercept demineralisation during active orthodontic therapy.
