Summary
Trigeminal neuralgia is a highly disabling craniofacial pain syndrome characterised by paroxysms of electric‐shock-like pain along one or more divisions of the trigeminal nerve. Management encompasses pharmacotherapy, interventional procedures and surgery, tailored to individual response and tolerability. First-line medical treatments are sodium-channel blockers, primarily carbamazepine and oxcarbazepine, with adjunctive use of lamotrigine, gabapentin, pregabalin or botulinum toxin in refractory cases. Minimally invasive interventions include trigger-point nerve blocks, high-voltage pulsed radiofrequency neuromodulation and rescue analgesic regimens for acute exacerbations. Surgical options range from microvascular decompression—preferred when neurovascular conflict is demonstrated on high-resolution imaging—to neuroablative techniques such as percutaneous Gasserian ganglion procedures and stereotactic radiosurgery. Emerging approaches investigate combination therapies, non-pharmacological modalities and biomarkers to predict treatment response. Optimising quality of life requires a multidisciplinary framework, balancing long-term efficacy, side-effect profiles and the patient’s risk tolerance for invasive treatments.
