Maternal Sleep Quality and Postpartum Mental Health

Summary

Maternal sleep quality undergoes significant disruption during pregnancy and the postpartum period, with implications for both physical recovery and psychological wellbeing. Sleep in new mothers is characterised by reduced total sleep time, increased nocturnal awakenings and fragmentation, and altered sleep architecture arising from hormonal fluctuations, infant care demands and psychosocial stressors. These disturbances can compromise mood regulation, cognitive performance and stress resilience, creating a bidirectional relationship between poor sleep and mood disorders. In particular, disrupted sleep has been linked to heightened risk of postpartum depressive symptoms, anxiety and obsessive–compulsive tendencies. Through mechanisms involving inflammatory pathways, hypothalamic–pituitary–adrenal axis activation and circadian misalignment, inadequate sleep may exacerbate underlying vulnerability to mood disturbance. Conversely, the onset of mood symptoms can further impair sleep continuity and quality, forming a self-reinforcing cycle. Recognising the global prevalence of sleep difficulties in the perinatal period—reported in both high-income and low-resource settings—public health strategies now emphasise early screening for sleep problems, education in sleep hygiene and non-pharmacological interventions such as cognitive behavioural therapy and light therapy. These approaches aim to restore healthy sleep patterns, enhance maternal resilience and reduce the burden of postpartum mental illness.

Postpartum depression: A mood disorder occurring after childbirth, marked by persistent low mood, anhedonia and functional impairment.
