Maternal Stress Impact on Childhood Allergic Diseases

Summary

Maternal stress during pregnancy and the early postnatal period has emerged as a significant factor in the developmental origins of allergic diseases in childhood. Evidence indicates that psychological distress—whether in the form of anxiety, depression, bereavement or job strain—can programme the developing immune system, shifting the balance towards atopic phenotypes such as asthma, wheeze and atopic dermatitis. These effects appear to be mediated by dysregulation of maternal neuroendocrine systems, notably the hypothalamic–pituitary–adrenal axis, and by epigenetic modifications in both mother and child. Observational cohorts spanning high-income and low- and middle-income settings report consistent dose–response relationships between higher stress levels and elevated risk of allergic outcomes, with critical windows during late gestation. The global burden of childhood asthma and eczema underscores the importance of maternal mental health support as a preventive strategy, with practical applications ranging from routine psychosocial screening in antenatal care to targeted interventions aimed at stress reduction and resilience building.

Recent studies have used genome-wide DNA methylation profiling to unravel the mechanistic link between prenatal psychosocial stress and persistent wheezing. Whole-genome bisulfite sequencing of mother–child pairs revealed stress-associated methylation changes predominantly in enhancer regions, affecting genes involved in neuroendocrine regulation and neurotransmitter receptor pathways. In children, but not in mothers, critical pathways for lung maturation—such as calcium and Wnt signalling—were epigenetically deregulated and correlated with persistent wheeze until age five. These insights highlight epigenetic reprogramming as a key mediator of stress-induced allergic susceptibility and point towards potential biomarkers and therapeutic targets to mitigate long-term respiratory morbidity.
