Mevalonate Pathway Dynamics in Cancer Biology

Summary

The mevalonate pathway is a central biosynthetic route that converts acetyl-CoA into cholesterol and nonsterol isoprenoids, supporting membrane architecture, protein prenylation and intracellular signalling. Dysregulation of this pathway is a hallmark of many malignancies, where elevated flux drives proliferation, metastasis and the maintenance of cancer stem cell populations. Cancer cells often upregulate 3-hydroxy-3-methylglutaryl-CoA reductase (HMGCR), the rate-limiting enzyme, and sterol regulatory element-binding protein 2 (SREBP2) to replenish intracellular sterols. Feedback loops triggered by statin drugs lead to compensatory enzyme induction, resulting in resistance. Mevalonate-derived intermediates contribute to oncogenic pathways, including small GTPase activation, YAP1 nuclear localisation and Hedgehog signalling, while high cholesterol levels modulate immune evasion via the cGAS–STING axis. Therapeutic targeting of this metabolic node utilises statins, bisphosphonates and novel inhibitors of pathway transcriptional regulators, alongside combination strategies with kinase inhibitors or immune modulators. Recent advances highlight the influence of diet and tumour microenvironment on mevalonate pathway dynamics, underscoring its global significance in cancer biology and therapy.

A foundational study has demonstrated that the dietary intake of geranylgeraniol, a mevalonate pathway intermediate, can negate the antitumour efficacy of a lipophilic statin in ovarian cancer models. This work revealed that geranylgeraniol supplementation restores mevalonate-derived prenylation and cell survival despite HMGCR inhibition, emphasising the need to control dietary precursors in clinical interventions. The findings also underscored the importance of statin pharmacokinetics, suggesting that long half-life agents combined with dietary regulation may improve therapeutic responses in statin-refractory ovarian tumours.
