Summary
The methylation status of the O6‐methylguanine‐DNA methyltransferase (MGMT) gene promoter is a pivotal predictive and prognostic biomarker in glioblastoma management. Epigenetic silencing of MGMT via promoter methylation compromises the tumour’s DNA‐repair capacity, thereby enhancing sensitivity to the alkylating agent temozolomide. Accurate, quantitative assessment of methylation at specific CpG sites—using techniques such as pyrosequencing or methylation‐specific PCR—enables stratification of patients into methylated and unmethylated groups, guiding treatment intensity and reducing unnecessary toxicity. Recent methodological refinements have identified critical regions within the promoter and adjacent enhancer elements whose methylation correlates strongly with overall survival. Incorporating combined promoter–enhancer methylation profiles into clinical workflows promises more precise prediction of therapeutic response and supports the development of risk‐adapted protocols worldwide.
