Micronutrient Supplementation Strategies in Early Childhood Nutrition
Summary
Micronutrient deficiencies remain a major obstacle to optimal growth, cognitive development and immune competence in children under five worldwide. Strategies to address these deficits encompass single‐nutrient supplements, multiple micronutrient powders (MNPs), lipid‐based nutrient supplements (LNS), large‐scale food fortification and point‐of‐use fortification. Single‐nutrient approaches traditionally target iron, vitamin A or zinc, while MNPs combine several essential vitamins and minerals in a sachet for home mixing with complementary foods. LNS deliver energy, protein and multiple micronutrients in a lipid matrix to support both growth and micronutrient status, often used in settings of severe acute malnutrition. Fortification of staple foods or condiments provides population‐level reach, though coverage and dietary patterns determine its impact. Evidence indicates that appropriately formulated MNPs and LNS can reduce anaemia and improve indices of nutritional status when integrated with promotion of optimal infant and young child feeding practices. However, programme effectiveness varies according to delivery platforms, adherence, baseline deficiency patterns and coexisting infections. Anthropometric outcomes such as weight‐for‐age and height‐for‐age z‐scores respond most robustly to LNS, whereas improvements in haemoglobin and micronutrient biochemical markers can be achieved through multiple modalities. Cost‐effectiveness analyses highlight the potential for scaled‐up home fortification to reduce the global burden of micronutrient malnutrition, especially when implemented alongside routine health contacts. Contextual factors—including dietary diversity, food security and caregiver education—mediate the success of any supplementation programme, underscoring the need for integrated policies that link supplementation, fortification and broader nutrition‐sensitive interventions.
