MicroRNA Biomarkers in Hepatocellular Carcinoma
Summary
Hepatocellular carcinoma (HCC) remains a major global health challenge, with high incidence and poor prognosis owing to late diagnosis and limited curative options. In recent years, microRNAs (miRNAs) have emerged as promising non-invasive biomarkers for early detection, prognostic stratification and monitoring of therapeutic response. These short, non-coding RNAs regulate key oncogenic and tumour-suppressive pathways, and their differential expression in blood, serum or plasma can reflect tumour burden and liver injury. Advantages of circulating miRNAs include remarkable stability, ease of quantification and potential for panel-based assays. Integration with existing markers such as alpha-fetoprotein (AFP) and application of machine learning for pattern recognition have further enhanced diagnostic accuracy. However, challenges remain in standardising sample processing, normalising data and validating findings across diverse patient populations. Continued efforts to harmonise protocols, define reference controls and explore exosomal versus free miRNA fractions are critical to translate miRNA biomarkers into routine clinical practice.

In a 2024 study investigating Egyptian patients with HCC, a machine learning framework was developed to evaluate circulating miRNAs as diagnostic and prognostic markers. Data preprocessing addressed heterogeneity across three independent cohorts, while a novel Binary African Vulture Optimisation algorithm selected key miRNA features. Support vector machine classification with cross-validation yielded sensitivities and specificities exceeding 95% in differentiating HCC from cirrhosis and healthy controls, highlighting miR-483-5p, miR-21 and miR-155 as particularly informative.
Foundational profiling efforts have employed whole-miRNome analysis to pinpoint deregulated miRNAs across multiple validation cohorts. Distinct signatures of upregulated miRNAs such as miR-122-5p and miR-125b-5p, together with sets of downregulated candidates, were shown to discriminate HCC from chronic hepatitis and cirrhosis. Associations between specific miRNA levels and overall survival further underscored their prognostic value, supporting their integration into risk stratification workflows.
