Microvascular Decompression in Hemifacial Spasm Management

Summary

Hemifacial spasm is a disabling movement disorder caused by vascular compression of the facial nerve at its root exit zone, leading to involuntary unilateral contractions of facial muscles. Microvascular decompression (MVD) offers a curative approach, aiming to relieve pulsatile contact by interposing a pad or repositioning offending vessels. Surgical success hinges on meticulous identification of neurovascular conflict, delicate mobilisation of arteries or veins—often the anterior inferior cerebellar, posterior inferior cerebellar or vertebrobasilar vessels—and protection of critical venous structures such as the sigmoid sinus. Intraoperative electrophysiological monitoring, notably the lateral spread response, guides decompression adequacy and predicts long-term outcomes. Advances in preoperative imaging and refined microsurgical techniques have reduced complication rates, including hearing loss and facial weakness, and enhanced durable spasm relief. Globally, MVD remains the treatment of choice for primary hemifacial spasm when botulinum toxin injections provide only temporary symptom control or when patients seek a lasting solution.

Stepwise escalation of stimulus intensity, with onset latencies longer than 10 ms, has been shown to reduce false positives and improve the reliability of abnormal muscle response interpretation during MVD. This refinement supports more accurate assessment of decompression completeness and may correlate with improved clinical outcomes. Separately, a comparative study of hemifacial spasm cases involving the vertebral artery has elucidated distinctive clinical features—such as more rapid symptom progression and increased operative complexity—and assessed the efficacy of an interposition technique. Despite higher rates of intraoperative auditory evoked potential changes, outcomes and overall complication profiles remain comparable to non-vertebral artery cases, supporting the feasibility of tailored surgical strategies in this subgroup.
