Summary

Dementia substantially shortens survival, with risk of death influenced by a constellation of demographic, clinical, functional and biological factors. Advancing age and male sex remain consistent predictors of higher all-cause mortality across dementia subtypes. Vascular comorbidities—particularly diabetes, hypertension and stroke—amplify risk, as do cancer and movement disorders in Alzheimer’s disease. Rapid cognitive decline and early functional disability independently heighten mortality, while preserved activities of daily living and slower progression confer relative protection. Neuroimaging markers of hippocampal and global cortical atrophy, alongside cerebrospinal fluid phosphorylated tau, further refine prognosis in Alzheimer’s cohorts. Glycaemic dysregulation has emerged as a modifiable risk domain, with poorly controlled or long-standing diabetes raising the hazard of death. Under-reporting of dementia on death certificates obscures its true mortality burden, complicating public health surveillance. Recent advances in predictive modelling enable personalised risk stratification, flagging patients for early palliative or intensive management. Together, these insights underscore the need for integrated care pathways that address metabolic, vascular and functional vulnerabilities in people living with dementia.

Recent predictive modelling studies have demonstrated that parsimonious machine-learning algorithms can flag dementia patients at high short-term mortality risk using a small set of clinical and neuropsychological features. By training gradient-boosted trees on national registry data, investigators achieved discrimination (area under the receiver operating characteristic curve) above 0.82 for 1-, 3-, 5- and 10-year survival thresholds. Stratified analyses revealed both shared and subtype-specific predictors, enabling targeted interventions. Unsupervised clustering of mortality drivers grouped vascular dementia with depressive symptoms and linked Lewy body dementia to frontotemporal atrophy patterns, highlighting distinct prognostic signatures across disease aetiologies.
