Muscle Energy Techniques in Managing Chronic Pain Conditions
Summary
Muscle Energy Technique (MET) is a form of manual therapy in which a practitioner guides a patient through specific isometric or isotonic muscle contractions against a controlled counterforce. Originating from osteopathic practice, MET aims to restore normal muscle length, improve joint mobility and enhance neuromuscular coordination. In the context of chronic pain conditions—such as non‐specific neck pain, low back pain and myofascial syndromes—MET is theorised to modulate pain via alterations in muscle spindle sensitivity, improved circulation and facilitation of endogenous analgesic pathways. Clinical protocols typically combine MET with stretching and other rehabilitative exercises to address muscular imbalances and joint restrictions. Recent investigations have explored optimal dosing parameters, the comparative efficacy of MET monotherapy versus combined approaches, and mechanistic underpinnings such as changes in muscle activation timing and postural control. Overall, MET has emerged as a globally utilised adjunct within multidisciplinary pain management, with particular appeal for its low risk profile and potential to promote self‐management strategies through patient engagement in active contractions.

Technical terms
Muscle Energy Technique (MET): A manual therapy method involving patient‐generated muscle contractions against a practitioner’s counterforce to normalise muscle length and joint function.
Isometric contraction: A muscle contraction in which tension develops without visible change in muscle length or joint movement.
Range of motion (ROM): The full movement potential of a joint, measured in degrees of a circle.
Centre of Pressure (COP): The point of application of the resultant vertical ground reaction force beneath the foot during balance and gait assessments.
