Summary

N-Acetylcysteine (NAC) is a derivative of the amino acid cysteine and serves principally as a precursor to the antioxidant glutathione. In the central nervous system, NAC has been shown to modulate redox balance, attenuate inflammatory signalling and influence glutamatergic neurotransmission. Preclinical studies indicate that NAC can protect neurons against oxidative damage, regulate microglial activation and counteract stress-related behavioural deficits. Clinically, NAC has been explored as an adjunctive therapy across a spectrum of psychiatric conditions, including schizophrenia, bipolar depression, obsessive-compulsive and related disorders, substance use disorders and post-traumatic stress disorder. Typical oral dosing ranges between 2 000 and 3 000 mg per day, with a favourable safety and tolerability profile. Despite promising results in terms of negative symptom reduction in schizophrenia and craving attenuation in addiction, outcomes in mood and trauma-related disorders remain mixed. Variability in trial design, patient selection and biomarker assessment underscores the need for well-powered studies with standardised endpoints. Globally, the potential to repurpose a readily accessible and low-cost agent such as NAC offers significant public health implications, particularly in low-resource settings where novel therapeutics are less available.
