Nanotechnology-based therapeutics have emerged as a transformative approach to address renal injury by exploiting the unique physicochemical properties of nanoscale materials. By engineering nanoparticles, drug conjugates and nanozymes with tailored size, surface charge and targeting ligands, researchers aim to enhance renal accumulation, overcome biological barriers and deliver antioxidants or bioactive agents directly to injured nephron segments. The primary focus has been on mitigating oxidative stress and inflammation, two central mechanisms driving acute kidney injury and chronic renal diseases. Strategies include antioxidant nanoclusters, organelle-targeted carriers and ligand-directed delivery to glomerular or tubular cells, offering improved biocompatibility, reduced off-target toxicity and the potential for real-time imaging guidance. Collectively, these advances point to a new era of precision nanomedicine for renal repair and functional restoration.

Recent studies have demonstrated the potential of antibody fragment drug conjugates to enhance renal targeting and therapeutic efficacy in diabetic nephropathy. One approach employed F(ab′)2 fragments directed against VEGFR2 linked to a mitochondria-targeted antioxidant peptide, resulting in superior kidney accumulation and glomerular repair compared with intact antibodies. Foundational work on polyoxometalate nanoclusters revealed that molybdenum-based nanoscale antioxidants preferentially localise in renal tissue, effectively scavenging reactive oxygen species and ameliorating acute kidney injury in animal models. In parallel, albumin-based nanoparticles have been used to deliver celastrol specifically to mesangial cells, thereby reducing systemic toxicity while alleviating proteinuria and glomerular inflammation in experimental glomerulonephritis.
