Nasopharyngeal Papillary Adenocarcinoma Characterization and Management
Summary
Nasopharyngeal papillary adenocarcinoma is a rare malignant epithelial neoplasm arising from the surface mucosa of the nasopharynx and characterised by delicate papillary structures lined by cuboidal to columnar cells. It often exhibits nuclear features resembling those of papillary thyroid carcinoma, including ground-glass chromatin and intranuclear grooves, yet lacks thyroglobulin expression. The tumour typically presents as a slow-growing exophytic mass, causing nasal obstruction, epistaxis or serous otitis media. Histopathological examination and immunohistochemical profiling—most notably positivity for thyroid transcription factor-1 and cytokeratin 19 alongside negativity for thyroglobulin and PAX-8—are essential to distinguish it from metastatic thyroid or salivary gland tumours. Endoscopic surgical excision with clear margins remains the mainstay of treatment, with precise radiotherapy reserved for incomplete resections or recurrent disease. Long-term outcomes are generally favourable, with low rates of regional metastasis and excellent disease-free survival, yet uniform staging criteria and adjuvant treatment guidelines are still evolving. Advancements in molecular profiling and refined surgical techniques have begun to inform tailored management strategies, underscoring the importance of multidisciplinary care in achieving optimal functional and oncological results.
