Summary

Chronic pain reflects not only persistent nociceptive input but also profound neuroadaptations within reward and motivational circuits. Prolonged noxious signalling engages ascending pathways from the spinal cord to thalamic and somatosensory regions while impairing descending modulatory systems that normally inhibit pain. Concurrently, mesolimbic structures such as the ventral tegmental area and nucleus accumbens undergo synaptic plasticity, altering dopaminergic and glutamatergic transmission. These changes shift motivational salience away from appetitive stimuli and contribute to anhedonia, mood disorders and maladaptive coping. Limbic and prefrontal networks integrating affective, cognitive and sensory dimensions become reorganised, promoting heightened pain sensitivity and negative affect. Understanding these intertwined processes has revealed that chronic pain and reward deficits share common molecular mediators, including receptor trafficking, neuroinflammatory signalling and altered neurotransmitter release. Insights into these mechanisms have spurred the development of circuit-targeted interventions aimed at restoring balanced reward–pain integration and alleviating both sensory and affective symptoms.

Recent studies have identified a specific glutamatergic pathway from the dorsal raphe nucleus to dopamine neurons in the ventral tegmental area that is inhibited by neuropathic pain. In animal models, chronic nerve injury suppresses glutamatergic drive onto midbrain dopaminergic cells, leading to diminished dopamine release in the nucleus accumbens and the emergence of anhedonia-like behaviours. Optogenetic restoration of this pathway not only alleviates sensory hypersensitivity but also reverses motivational deficits, demonstrating its dual role in modulating both pain perception and reward processing. This work highlights precise circuit nodes that may be harnessed for targeted neuromodulation to treat the comorbid sensory and affective dimensions of chronic pain.
