Neuroimaging and Treatment Mechanisms in Major Depressive Disorder
Summary
Major Depressive Disorder (MDD) is a prevalent, disabling condition characterised by persistent low mood, anhedonia and cognitive disturbances. Neuroimaging modalities such as functional magnetic resonance imaging, diffusion tensor imaging and positron emission tomography have elucidated dysregulation within distributed brain networks, including the default mode, salience and frontoparietal attention circuits. These studies have revealed both structural alterations in grey and white matter and functional changes in intrinsic connectivity and task-evoked activation. Converging evidence suggests that early modulation of negative emotional bias and restoration of top–down cognitive control are key mechanisms underlying clinical improvement. Contemporary research aims to stratify patients into biologically informed subgroups, to predict individual treatment response to pharmacotherapy, psychotherapy and neuromodulation, and to refine targets for precision interventions in MDD.
Research from Nature Portfolio
Two recent investigations have advanced mechanistic understanding of MDD through multimodal neuroimaging and causal manipulation. One study derived personalised scores of intrinsic and task-evoked functional connectivity to parse six clinically distinct biotypes of depression and anxiety, each defined by unique patterns of default mode, salience and frontoparietal circuit dysfunction and corresponding differential response to pharmacological and behavioural interventions. A second investigation in non-human primates employed microstimulation of the subgenual cingulate cortex and concurrent local field potential recording to demonstrate that disruption of top–down influence from dorsolateral prefrontal regions induces a negative decision-making bias, illuminating a causal pathway between cortical regulation and limbic activity that may underlie depressive cognition.

Salience network: A neural circuit, including anterior insula and cingulate cortex, responsible for detecting and filtering behaviourally relevant stimuli.
Microstimulation: The application of electrical stimulation to specific brain regions to causally modulate neural activity and assess its behavioural effects.
Biotypes: Distinct subgroups of patients characterised by reproducible neurobiological profiles, enabling stratification beyond symptom classification.
