Summary

Neuroimaging of sexual arousal and dysfunction has matured into an interdisciplinary field that integrates functional and structural mapping to elucidate the neural underpinnings of normal and disordered sexual behaviour. Functional magnetic resonance imaging (fMRI) measures haemodynamic responses during exposure to erotic stimuli or at rest, while structural MRI assesses grey matter alterations in regions associated with desire, reward and motivation. Core circuits involve the insula, anterior cingulate and orbitofrontal cortices, hypothalamus, ventral striatum and thalamic nuclei. High-field imaging has enabled resolution of subcortical nuclei and graph-theoretical analyses characterise network efficiency and topology. In psychogenic erectile dysfunction, aberrant baseline activity and disrupted connectivity in prefrontal, striatal and limbic nodes correlate with clinical measures of sexual function and affective state.
